Patient meets all criteria following observation?
Normal mental status
Resolving or minor symptoms
Tolerating oral intake
Dependable social support
No suspicion of abuse or neglect
Normal CT
OR
Non-depressed or minimally depressed skull fracture
AND
NO intracranial hemorrhage
AND
> 6 months of age
Observe 3-4 hours
OR
Obtain head CT
< 2 years of age? (2-4 see supplemental document)**
Severe mechanism of injury 2
High risk hematoma 3
LOC > 5 seconds
Vomiting 4
> 2 years of age?
Severe mechanism of injury 2
Severe headache
Any loss of consciousness
Vomiting 4
GCS 14 or 15
Altered mental status
Seizure
GCS <13 or altered mental status
AND/OR
Obvious markedly depressed or open skull fracture or evidence of basilar skull fracture
AND/OR
Focal neurologic exam
